About the event
Advance Care Planning (ACP) involves thinking, documenting, and discussing healthcare preferences and decisions in advance of a serious illness or end-of-life care. In this interactive four-part workshop, individuals will be equipped with the essential information to complete their Advance Directive document and discuss these plans with their loved ones. By the end of the workshop, participants will feel empowered to take the next steps toward proactive, meaningful planning.
For best results, it is recommended that you attend all four sessions:
Session 1: Wednesday, November 5, 12:00-1:30
Session 2: Thursday, November 20, 1:00-2:30
Session 3: Thursday, December 4, 1:00-2:30
Session 4: Thursday, December 18, 1:00-2:30
