About the event
Advance Care Planning (ACP) involves thinking, documenting, and discussing healthcare preferences and decisions in advance of a serious illness or end-of-life care. This interactive, 4-session workshop equips individuals with the essential information to complete their Advance Directive document and helps them discuss these plans with their loved ones. By the end of the course, participants feel empowered to take the next steps toward proactive, meaningful planning. For best results, it is highly recommended to attend all four sessions.
